This Gift Card (this “Card”), issued to you by LA Fitness, may be used to purchase fitness or personal training (“PT”) memberships (but not other gift cards, other services, or goods) offered by LA Fitness. This Card is not reloadable. After initial redemption, any remaining balance on the Card: (a) is non-transferable; and (b) will be applied against Cardholder’s subsequent billing cycle(s) (or renewal) until depletion. LA Fitness makes no representations, warranties, or conditions of any kind with respect to this Card. Except as required by law, any amount on this Card is non-refundable and may not be redeemed for cash. Purchases made using this Card are non-refundable, except as set forth in your fitness or PT agreement. The value of this Card will not be replaced if this Card is lost, stolen, destroyed, altered, or used without permission. No expiration date.
